Leave of Absence: Steps for employees under UTime
 

  1. Complete and follow the instructions on Request Leave of Absence - UTime Form

  2. 联系 Sun Life to initiate your leave request (instructions are on the 太阳生命形式)

What you can expect during your Leave of Absence:
 

  • 第一步: 完成 Request Leave of Absence - UTime Form 30 days in advance of your leave request begin date and submit to your direct supervisor / chair for review and processing. 你的 direct supervisor / chair must provide the Request Leave of Absence - UTime Form to 人力资源的好处 which will allow for your benefits to be researched. You will be notified of your leave benefits and any additional information that is needed.  It is understood this advance notice may not be possible for unforeseen leaves and/or emergencies.  It is your responsibility to ensure that 人力资源的好处 receives this information.
     

  • 第二步: 联系 Sun Life to initiate your leave request (instructions are on the 太阳生命形式)
     

  • 第三步: Stay in touch!  The 人力资源的好处 team and Sun Life will communicate with you in writing to verify your leave status and to notify you of any additional documentation needed.  你的 HR Partner and Direct Chair / Supervisor will be copied in each letter from 人力资源的好处 only, to keep them informed.

A Leave of Absence is broken into 2 categories:
 
  • 类别1: What type of Leave you are on.
    It is best to schedule an appointment with the 人力资源的好处 team to discuss the type of leave that applies to your request. You may find that you are on more than one type of leave at the same time.
  • 你的 Leave of Absence (LOA) will be designated based on your eligibility.   Below are the different types of LOA's. Not all employees are eligible for all types:

军事 家庭 军事 个人 家庭护理
亲子鉴定 Jury Duty or Witness 家庭 & 医疗 Leave (FMLA) Workers' 补偿
 
  • 类别2:  How you are paid during your leave.
    UNH has 带薪休假 benefits and whether you are paid or not during your leave depends on the 带薪休假 benefits that are available to you.
